Reporting hazards, incidents or injuries

Use the form below  in all instances where injury, damage or a near miss has occurred: 

• Hazard Incident Injury Report Form

It is important this form be completed as soon as practicable after the event to enable Indigo Shire to remedy any hazardous situations as a matter of priority.
